</div>회사원 박모(35)씨는 허리 통증으로 최근 병원을 찾았다가 가벼운 추간판탈출증(디스크) 진단을 받았다. MRI를 찍은 비용 50만원이 들었지만 지난 5월 가입한 실손보험 MRI 특약으로 35만원을 보상받았다.

<p dmcf-pid="LSVfwp8MUw" dmcf-ptype="general">박씨처럼 증상의 진행 상황을 지켜보기 위해 MRI를 찍어야 하는 디스크 환자들은 그동안 실손보험을 유지할 수밖에 없었다. 하지만 박씨는 내년에도 MRI 특약을 유지해야 할지 고민하고 있다. 이르면 올해 말부터 디스크와 관련한 MRI 촬영에 건강보험이 적용되기 때문이다.</p>
<p dmcf-pid="LsAPPJRi4x" dmcf-ptype="general">정부가 9일 ‘건강보험 보장성 강화대책’을 발표한 뒤 실손보험 가입자들의 계산이 복잡해졌다. 이번 대책에 따르면 2022년까지 치료에 필수적인 비급여(환자 본인 부담 100%)에 단계적으로 건강보험이 적용된다. 실손보험의 효용성에 대한 의문이 커지게 된 것이다. 실손보험은 건강보험의 비급여 항목과 급여 항목 중에서 본인부담금을 보장하는 상품이다. 전체 진료비에서 건강보험이 보장해 주는 비율은 지난해 63.4%에 불과했다. 나머지는 개인 몫이다. 특히 중증질환으로 인한 고액 의료비 부담의 상당 부분을 개인이 떠안으면서 실손보험은 ‘국민보험’이 됐다. 금융감독원에 따르면 지난해 말 현재 실손보험 가입 건수는 3266만 건이다. 전 국민의 65%가량이 실손보험을 가지고 있는 셈이다.</p>

실손보험 유지와 해지를 가를 가장 중요한 변수는 가입 시점이다. 실손보험 보장 표준화가 이뤄진 2009년 10월 이전 가입자는 보험을 유지하는 편이 낫다. 비급여 항목의 경우 입원(100% 보장)은 자기부담금이 없고 통원의 경우 5000원 정도만 내면 되기 때문이다.

</div> 2009년 10월 이후 가입자의 손익 계산은 복잡하다. 손해보험협회 주진현 대리는 “몇 차례의 보장 표준화 작업 이뤄지면서 자기부담금 수준이 조정된 만큼 가입 시기에 따라 이번 대책의 영향이 다를 수밖에 없다”고 말했다. 2009년 10월 이후부터 2012년까지 실손보험에 가입한 경우 비급여 항목의 자기부담금(입원의 경우)은 10% 수준이다. 2013년 1월부터 지난해 3월까지 가입자의 경우 자기부담금을 10%와 20% 사이에서 선택할 수 있었다. 지난 4월 도수치료와 MRI·주사 등을 특약으로 바꾼 이후 가입한 경우는 더 복잡해질 수 있다.

<p dmcf-pid="LmZBbntSDu" dmcf-ptype="general">보건복지부는 3800여 개의 비급여 항목을 건보 대상에 포함하지만 환자가 내는 본인부담금의 비율은 30~90%로 차등화하겠다고 밝혔다. 이렇게 되면 보험 가입자가 질환별로 건강보험이 적용되는 시기와 본인부담금을 따져 갱신 여부를 결정해야 한다. 본인이 아픈 질병에 건강보험이 적용되더라도 본인부담금이 일부 남아 있는 만큼 섣불리 실손보험을 해지해선 안 된다는 얘기다.</p>
<p dmcf-pid="LQIJGMWqSN" dmcf-ptype="general">보험업계 관계자는 “건강보험이 적용될 질환이나 진료가 아직 결정되지 않은 데다 본인부담금의 비율도 제각각(30~90%)일 수 있는 만큼 실손보험 가입자의 경우 당분간 현재의 보험을 유지하면서 정부의 방안을 지켜본 뒤 실손보험 유지 여부를 결정하는 것이 더 나을 것”이라고 말했다. 또한 진단자금을 지급하는 생명보험의 건강보험(암 보험이나 치매 보험) 등은 이번 대책과 무관한 만큼 유지하는 것이 낫다.</p>
<p dmcf-pid="L7snnmPpwE" dmcf-ptype="general">메리츠종금증권의 김고은 애널리스트는 “2022년까지 건강보험 보장률 70%를 달성한다는 것은 민간 보험이 담당해야 할 영역이 남아 있다는 의미”라고 지적했다.</p>
<p dmcf-pid="L8WGVNxh5a" dmcf-ptype="general">건강보험의 적용 확대로 실손보험이 위축될 것이란 우려에도 보험사는 일단 환영하는 분위기다. 비급여 부분에서 벌어졌던 과잉 진료의 억제를 기대할 수 있어서다. 일부 환자의 ‘의료 쇼핑’과 실손보험 가입자들에게 고가의 비급여 진료를 권유한 의료기관 등으로 인해 실손보험의 손해율(보험사가 받는 보험료 대비 지급하는 보험금의 비율)이 계속 증가했다. 금융감독원에 따르면 2012년 112.5%이던 손해율은 2015년 122.1%까지 상승했다. SK증권에 따르면 지난해 말 기준으로 삼성화재를 제외한 주요 손해보험사의 경우 130%의 높은 실손보험 손해율을 기록했다. 김도하 SK증권 애널리스트는 “과잉진료 억제로 손해율 정상화에 기여할 수 있을 것”이라며 “건강보험 적용 범위 확대가 실손보험료 인하 요인으로 작용할 수 있을 것”이라고 말했다.</p>
